Description: This recorded webinar discusses how health centers can benefit from sharing data with housing providers, and includes both a community example of such work in action and a presentation from law firm Akin Gump and the Center for Medical Legal Partnership on how to approach data sharing agreements, keeping in mind HIPAA privacy restrictions. The majority of health and housing collaborations use a data driven methodology to identify “frequent users.” But data is useful beyond the identification phase – sharing health center data is also critical to referral, care transition, and care coordination with housing providers. Sharing data helps to not only improve care for patients but also to measure the success of such initiatives in terms of both health outcomes and cost savings.
